Relocation expenses included in hire package if needed

Excellent beginning salary (based on education and experience), Healthcare, and Vacation pay.

Work Hours: Monday thru Friday - 8:30 - 12:00 and 1:00 - 4:00.

Expectations:

• Sign non-VNA orders
• Dual-care practices
• Respond to incoming calls, letters, and faxes - same day turn around
• Some walk-ins accepted
• Established patients
• Accepting new patients
• Well-documented progress notes - warranting a CPT code
• Strict pharmacy formulary
• Maintain EDRP, JCAHO, Care Line, and facility standards and policies
• Must be a professional care team member
• CPRS - Computerized Patient Record System - all records signed within 24 hours
• Must meet all agency credentialing requirements
• Must be privileged as an Independent Provider
• Hold a Masters Degree in Nursing
